THE PURPOSE OF HEALTH SCREENS
The purpose of offering health screens is for everyone to receive baseline information on their current health status. The Biometric screening tests include fasting levels of: CBC w/ Autodiff, Sodium, Potassium, Albumin, Alkaline, Phosphatase, Bilirubin, BUN, Calcium, Creatinine, GGT, Globuline,AST, ALT, Total Protein, Uric Acid, LOH, Glucose, CHolesterol, HDL, LDL, VLDL, Triglycerides, TC/HDL ratio, and HbA1c.
We will have the In-Body available and optional for anyone that would like to have the test done. The benefit of completing your Biometric Health Screen and CHRA is that anyone that is currently on our insurance will be eligible to save $30/month on their insurance premium. To receive the discount for the current wellness year, you must complete the following between January 1st and March 31st of the current wellness year.

Health Screening/Biometrics
The Health Risk Assessment (CHRA online questionnaire) is available online starting February 1st of the current wellness year for staff receiving District insurance. For staff not electing District insurance, the CHRA will be available to complete beginning April 1st of the current wellness year.
You may also use your general practitioner to complete your biometric health screen. For this, refer to the Physician Lab Form. This must be completed and submitted to UMR, dated between January 1st and March 31st of the current wellness year. Completion of the health screen outside this window will not be accepted in place of the biometric blood draw.
Clinical Health Risk Assessment (CHRA)
The health risk assessment will be open starting February 1, 2025. It will close March 31, 2025. Please go to www.umr.com to take your CHRA survey.
The deadline for completing CHRA is March 31, 2025. If you have not completed your CHRA on or before the deadline, you will not be eligible for incentives. There are no extensions available due to time constraints of processing. Please reach out to your building champion for help completing your CHRA if you need help.
EXEMPTIONS FROM SCREENS:
Anyone that is pregnant – (Still required to take CHRA online)
Anyone undergoing medical treatment that would affect the results of the biometric screens (Still required to take CHRA online)

COMMON FAQ’s/Questions about the screens:

Do I need to fast? Yes, for a minimum of 8 hours prior to the test. You may have a glass of water.

If I’m pregnant, should I complete the screen? No, you will only take the online Health Risk Assessment. You will need to email Wesley Lochhead to be added to the exemption list.

If this is my first year at JC Schools and I took the CHRA in August - October, do I need to retake the CHRA? Yes, the CHRA and labs are to be completed to give you the discount and ability to participate in the $200 Wellness Reward for the current wellness year.

If I recently had blood work done, can I use it? Yes, if you have had a blood draw/workup completed after January 1st and before March 31st of the current wellness year. You will need to have all other items completed as well and use the UMR Physician lab form.

What is involved with the screen? Blood draw is the only requirement.
Please come at your selected time and have the consent form and proper paperwork filled out prior. The health screens are designed to serve up to 15 staff in roughly 30-minute increments. If you come too early or too late, it will make the lines longer.
